Ivermectin's FDA-approved uses — strongyloidiasis and onchocerciasis (river blindness) — reflect its core competency: killing parasites. That part isn't in dispute. What's contested is how common parasitic infection actually is in places like the US, and how much of it goes undiagnosed.

The numbers are noteworthy.

Estimates cited in the parasitology literature also point to smaller but real populations affected by Chagas disease and cysticercosis. A 2023 review in Clinical Microbiology Reviews argues these infections are underrated as a domestic public health issue, not just a "travel medicine" concern. The CDC has also flagged neglected parasitic infections as an area where diagnostic awareness lags the actual burden.

At the same time, many public health researchers push back on the notion that "most" Americans are walking around with an undiagnosed parasite load driving their fatigue, brain fog, or chronic symptoms. In their opinion, the data don't support that version.

They believe parasitic infection in the US is real, likely underdiagnosed in specific populations (immunocompromised patients, certain immigrant communities, people with specific exposures), and probably not the hidden epidemic some claim.

Both things can be true at once, and reasonable people land in different places on how much weight to give each side.

The Cancer Question: Real Research, Very Early Stage

This is the part getting more attention lately, and it's worth being precise about what's actually been shown versus what's being implied.

The mechanistic case starts with a 2014 study in EMBO Molecular Medicine by Melotti et al., which found that ivermectin inhibits the WNT-TCF signaling pathway — a pathway implicated in a wide range of cancers — and induced apoptosis in several human cancer cell lines in vitro, with selective effects in mouse xenograft models.

A 2018 review in the American Journal of Cancer Research ("The multitargeted drug ivermectin: from an antiparasitic agent to a repositioned cancer drug") catalogs a broader set of mechanisms proposed across the literature: induction of reactive oxygen species and mitochondrial dysfunction, modulation of the Akt/mTOR pathway, and effects on cancer stem cell populations, studied across glioma, colorectal, bladder, cervical, breast, and other cell lines. More recent papers, including a 2025 piece on glioblastoma repurposing and one on endocrine-resistant breast cancer via Wnt signaling, keep extending the list of cancer types studied in vitro.

Here's where it's important to slow down. As of mid-2026, a PubMed search across ivermectin-and-cancer literature turns up roughly 50 preclinical (lab and animal) studies and effectively no completed human clinical trials demonstrating a therapeutic benefit. There's exactly one active human trial that's gotten notice — a Phase I/II study at Cedars-Sinai combining ivermectin with checkpoint inhibitors (Balstilimab or Pembrolizumab) in metastatic triple-negative breast cancer — and it hasn't reported results.

Multiple recent review articles, including a 2025 piece in Current Oncology Reports and separate commentary flagged by the National Cancer Institute's own researchers, raise a specific technical concern: the concentrations of ivermectin needed to kill cancer cells in a petri dish are often at or above what's achievable in human blood plasma at safe doses. That gap — lab concentration versus achievable human dose — is the central unresolved question in this entire body of research, and it's the reason "kills cancer cells in vitro" and "treats cancer in people" are not the same claim.

The mechanistic story is genuinely interesting, multiple independent labs have published on it, and it's attracted enough legitimate scientific interest to generate an NCI look and at least one real oncology trial. It's also, right now, unproven in humans, with a known dosing problem that hasn't been solved. Both facts are true simultaneously, and where that leaves things depends on how much weight you put on early mechanistic signal versus the absence of clinical proof.

Where This Leaves Things

We're not going to tell you there's a clear answer here, because there isn't one yet — not on how big the parasite problem really is, and not on whether the cancer research pans out in people.
