What is Wellness Care?
Wellness Care is a Medicare-covered care management program that gives eligible patients extra support with their ongoing health needs without replacing the doctors they already know and trust.
The program can include Chronic Care Management (CCM), Transitional Care Management (TCM), and Remote Patient Monitoring (RPM) with included devices like blood pressure cuffs, scales, and glucometers.
Your care team receives your readings to help track changes and support better management of your condition(s).

Stay connected with your care team between appointments. Real conversations with real people. No AI chatbots. Get help coordinating with your doctors, accessing helpful medical devices, and knowing what to do when something changes.

Yes. Chronic Care Management (CCM) is an approved Medicare program. If you qualify, Medicare pays for it. In some cases there may be a small monthly copay depending on your plan. We’ll check that for you before you enroll so there are no surprises.
