It Starts with Your Care Pro

Your Care Pro acts as your personal health concierge, providing individualized assistance to help you achieve your health goals. This is a dynamic, evolving, and truly holistic care model that puts you in the driver’s seat.

Once your RN care manager, doctor, and others in allied health fields have set your care plan in motion, your care pro:

  • Partners with you and your RN care manager to set realistic, achievable goals by acting as a liaison between professions.
  • Meets with you on a regular basis to monitor and report to your RN care manager any changes or concerns that may arise.
  • Gathers information and identifies barriers to follow-through on goals set forth in the care plan.
  • Coaches and educates on self-management support and preventative care.
  • Conducts health and wellness coaching sessions to make lasting changes in lifestyle and prevention.
  • Assists in finding the answers to health insurance questions and options
  • Connects you to services and supports you may need, such as community and healthcare resources, assistance in the home, and transportation needs
  • With RN supervision, supports transitions of care if discharged from the hospital, rehab, or emergency department. By doing so, you are assured you won't miss appointments, medication changes, or therapies that were ordered.
  • Identifies gaps in care and serves as a liaison between facilities and other entities to prevent delays and provide continuity of care.
  • Source healthcare professionals in compliance with your care plan, such as:
    • Diabetic or other specialized diet counselling,
    • Physical or occupational therapy,
    • Mental health resources,
    • Pain management,
    • Sleep specialist,
    • Exercise program or activity suited to your ability
