It showed up in my feed one afternoon — a short explainer about a Medicare
benefit most seniors had never heard of.
It walked through something called Chronic Care Management, and what it
said stopped me cold:
- "If you have two or more ongoing health conditions, Medicare has already
authorized monthly care support — at little to no cost to you."
- "You don't need a new doctor. You don't need to leave your house."
- "A dedicated care team calls you. You don't have to chase them."
I'd had Medicare for six years. No one had ever mentioned this to me.
I watched it twice. Then I called the number.
