Before your appointment time, please ensure that you have completed the following steps:

1. Patient Portal Registration: If you have not registered yet, check your email for an invitation to join our Patient Portal account. Use the link provided in the email to complete your registration. Please complete this step before your appointment.

2. Intake Form/Questionnaire: You will receive an email containing a link to access the general intake form/questionnaire on your Patient Portal. Please complete this form before your appointment.
Note: Both the Patient Portal registration and the completion of the intake form/questionnaire are required prior to your appointment time.

- After completing the registration process and filling out your intake/questionnaire forms, please locate your "Appointment Confirmed" or "Appointment Reminder" email.
- Click on the provided link to access your appointment. If clicking on the link does not work, you can manually copy and paste it into your web browser.

- You can join your session 30 minutes before your scheduled appointment time.

1. Enter your Full name under the "Attendee Name" field.

2. Select your state of residence from the dropdown menu.

3. Click, "Join" to accept the ChARM TeleHealth Terms of Service and enter the session.

- To complete your telehealth service, your provider needs to confirm your identity. Please follow these steps to upload your ID:

- 1. Select the ID Type that is most convenient for you from the dropdown menu.
- 2. Enter your ID# in the designated field.
- 3. Scan or take a picture of your ID and save it on your device. Then, click "Choose File" to locate and attach your saved ID file.
- 4. Click "Upload" to submit your identification.

STEP 5: Attending your Appointment
- Once you enter the virtual waiting room, your doctor will initiate the appointment at the scheduled start time. Please wait for your doctor to join the session and begin the consultation.
